What to Expect
We understand that finding a new provider can be difficult. Our staff is here to support you and find the provider that best meets your needs. To get started, give us a call or text at 402-898-1113 and a front desk staff member will guide you.
Our providers require all paperwork to be completed prior to scheduling. You can find these forms here (link) or our office staff can email/text the forms to you directly. Once finished, our front desk staff will give you a call to schedule.
Your provider may send extra screeners for you to complete prior to your appointment.
We encourage all patients to check with your insurance for benefits and eligibility before seeing our providers.
The first appointment will focus on gathering information and treatment planning, and to establish if it is a good fit.
