What to Expect

We understand that finding a new provider can be difficult. Our staff is here to support you and find the provider that best meets your needs. To get started, give us a call or text at 402-898-1113 and a front desk staff member will guide you.


  • Our providers require all paperwork to be completed prior to scheduling. You can find these forms here or our office staff can email/text the forms to you directly. Once finished, our front desk staff will give you a call to schedule. 

  • Your provider may send extra screeners for you to complete prior to your appointment.

  • We encourage all patients to check with your insurance for benefits and eligibility before seeing our providers. 

  • The first appointment will focus on gathering information and treatment planning, and to establish if it is a good fit.
